Discharge to Assess — Commissioning Proforma
KupaCare operates a clinically governed Discharge to Assess (D2A) and step-down reablement pathway, designed to support ICBs, local authorities, and NHS trusts in reducing delayed transfers of care (DTOC) and winter pressure bed-blocking across the Gwent Regional Partnership area.
Packages are time-limited, outcome-focused, and reviewed at agreed milestones in collaboration with the referring MDT. All delegated healthcare tasks are delivered under a clinical governance framework with formal reporting to the commissioning body.

Maximum Weekly Contribution (Wales Cap)

Local authority top-up rules apply where commissioned directly.
